Yes you can leave it plugged in all the time. When the phone isn't connected, it's smart enough to realize it's not in use. When you connect your phone a blue light on the charger indicates current is going to the phone. So there is no risk to the car or device.
Each USB port supplies 2.4 amps. If you only need or want to charge one device get the one port model.
It's 2.4 for each port. I don't think you will get 4.8 out of any port on any car charger.
The solid state electronics in this adapter can deal with either 12-14V or 24-28V. The system is rated at 12V or 24V (cars are almost always 12V), but alternators generate a higher voltage in order to recharge the battery as well as run the electrical system. 24V systems are found in buses, boats, trucks, airplanes, etc...
