I had a hard time finding something with all the items I wanted. This was the one. Lighted Switch - check USB outlets - check Rear mounting slots - check Thin profile - check Black color - check I mounted this on the side of my headboard and works great to charge my gadgets and gives me access to outlets without crawiing behind my bed.
